רִבּוֹנוֹ שֶׁל עוֹלָם, מַלֵּא מִשְׁאֲלוֹת לִבִּי לְטוֹבָה,
וְהָפֵק רְצוֹנִי וְתֵן שְׁאֵלָתִי לִי עַבְדְּךָ בֶּן
אֲמָתֶךָ (פְּלוֹנִי) בֶּן (פְּלוֹנִית), וְזַכֵּנִי (וְאֶת אִשְׁתִּי\
וּבָנַי\וּבְנֵי בָנַי\וּבְנוֹתַי) לַעֲשׂוֹת רְצוֹנְךָ בְּלֵבָב
שָׁלֵם, וּמַלְּטֵנוּ מִיֵּצֶר הָרָע, וְתֵן חֶלְקֵנוּ בְּתוֹרָתֶךָ,
וְזַכֵּנוּ שֶׁתִּשְׁרֶה שְׁכִינָתְךָ בְּתוֹכֵנוּ, וְהוֹפַע עָלֵינוּ
רוּחַ חָכְמָה וּבִינָה, וְיִתְקַיֵּם בָּנוּ מִקְרָא שֶׁכָּתוּב:
וְנָחָה עָלָיו רוּחַ יְיָ, רוּחַ חָכְמָה וּבִינָה, רוּחַ עֵצָה
וּגְבוּרָה, רוּחַ דַּעַת וְיִרְאַת יְיָ:
וְכֵן יְהִי רָצוֹן מִלְּפָנֶיךָ יְיָ אֱלֹהֵינוּ וֵאלֹהֵי
אֲבוֹתֵינוּ, שֶׁתְּזַכֵּנוּ לַעֲשׂוֹת מַעֲשִׂים טוֹבִים
בְּעֵינֶיךָ, וְלָלֶכֶת בְּדַרְכֵי יְשָׁרִים לְפָנֶיךָ,
וְקַדְּשֵׁנוּ בְּמִצְוֹתֶיךָ, וְנִזְכֶּה לְחַיִּים טוֹבִים
וַאֲרוּכִים, וּלְחַיֵּי הָעוֹלָם הַבָּא, וְתִשְׁמְרֵנוּ
מִמַּעֲשִׂים רָעִים וּמִשָּׁעוֹת רָעוֹת הַמִּתְרַגְּשׁוֹת
לָבֹא לָעוֹלָם, וְהַבּוֹטֵחַ בַּיְיָ חֶסֶד יְסוֹבְבֶנְהוּ,
אָמֵן:
יִהְיוּ לְרָצוֹן אִמְרֵי פִי וְהֶגְיוֹן לִבִּי לְפָנֶיךָ,
יְיָ צוּרִי וְגוֹאֲלִי:
וַאֲנִי תְפִלָּתִי לְךָ יְיָ, עֵת רָצוֹן, אֱלֹהִים בְּרָב
חַסְדֶּךָ, עֲנֵנִי בֶאֱמֶת יִשְׁעֶךָ:
וְהָפֵק רְצוֹנִי וְתֵן שְׁאֵלָתִי לִי עַבְדְּךָ בֶּן

רוּחַ חָכְמָה וּבִינָה, וְיִתְקַיֵּם בָּנוּ מִקְרָא שֶׁכָּתוּב:
וְנָחָה עָלָיו רוּחַ יְיָ, רוּחַ חָכְמָה וּבִינָה, רוּחַ עֵצָה
וּגְבוּרָה, רוּחַ דַּעַת וְיִרְאַת יְיָ:
וְכֵן יְהִי רָצוֹן מִלְּפָנֶיךָ יְיָ אֱלֹהֵינוּ וֵאלֹהֵי
אֲבוֹתֵינוּ, שֶׁתְּזַכֵּנוּ לַעֲשׂוֹת מַעֲשִׂים טוֹבִים
בְּעֵינֶיךָ, וְלָלֶכֶת בְּדַרְכֵי יְשָׁרִים לְפָנֶיךָ,
וְקַדְּשֵׁנוּ בְּמִצְוֹתֶיךָ, וְנִזְכֶּה לְחַיִּים טוֹבִים
וַאֲרוּכִים, וּלְחַיֵּי הָעוֹלָם הַבָּא, וְתִשְׁמְרֵנוּ
מִמַּעֲשִׂים רָעִים וּמִשָּׁעוֹת רָעוֹת הַמִּתְרַגְּשׁוֹת
לָבֹא לָעוֹלָם, וְהַבּוֹטֵחַ בַּיְיָ חֶסֶד יְסוֹבְבֶנְהוּ,
אָמֵן:
יִהְיוּ לְרָצוֹן אִמְרֵי פִי וְהֶגְיוֹן לִבִּי לְפָנֶיךָ,
יְיָ צוּרִי וְגוֹאֲלִי:
וַאֲנִי תְפִלָּתִי לְךָ יְיָ, עֵת רָצוֹן, אֱלֹהִים בְּרָב
חַסְדֶּךָ, עֲנֵנִי בֶאֱמֶת יִשְׁעֶךָ:

ve-zakkenu she-tishreh Shekhinatkha betokhenu, ve-hofa aleinu
ruach chochmah u-vinah, ve-yitqayyem banu mikra shekatuv:
ve-nachah alav ruach Adonai, ruach chochmah u-vinah, ruach etzah
ugvurah, ruach da'at ve-yir'at Adonai:
vechen yehi ratzon milfanekha Adonai Eloheinu veilohei
avoteinu, she-tezakkenu la'asot ma'asim tovim
b'einecha, ve-lalekhet be-darkhei yesharim lefanecha,
ve-qaddeshenu bemitzvotecha, ve-nizkeh lechayim tovim
va-arukhim, u-le-chayyei ha-olam haba, ve-tishmerenu
mi-ma'asim ra'im u-mi-sha'ot ra'ot ha-mitraggeshot
lavo la-olam, ve-ha-boteach ba-Adonai chesed yesovevenhu,

English
God, Master of the universe, fulfill the requests of my heart for the good. Satisfy my wishes and give what I ask for, to me—Your servant the son/daughter of Your maidservant, [your name] the son/daughter of [your mother’s name]—and give me (add as appropriate: and my wife/son/sons/daughter/daughters) the privilege to do Your will whole-heartedly. Save us from the evil inclination, and give us the ability to learn our portion in Your Torah. Give us the privilege of having Your Divine Presence rest within us. Bring a spirit of wisdom and understanding upon us, and may the following verse, written in Isaiah, be fulfilled through us: And the spirit of the Lord will rest upon Mashiach—the spirit of wisdom and understanding, the spirit of good advice and strength, and the spirit of knowledge and fear of the Lord. And so may it be Your will, Lord, our God and the God of our fathers, that You should give us the privilege to do deeds that are good in Your eyes and to walk before You in the paths of those who are upright and righteous. Make us holy through Your commandments, so that we will be privileged to have good and long life in this world and to have life in the World to Come. Guard us from bad deeds and bad times that may come to the world. Fulfill that which is written in Psalms: Kindness will surround the person who trusts in the Lord. Amen. May the words of the prayers that come from my mouth, and the thoughts of my heart, be acceptable to You—Lord, my Strength and my Redeemer. When I say my prayer to You, Lord, it should be at a special time, when prayers are listened to. God, with Your great kindness, answer me with Your true and everlasting salvation.
