ERUV TAVSHILIN
Should a festival fall on Thursday and Friday, or on Friday and Shabbat, an Eruv Tavshilin should be made on the day preceding the festival; This is done by taking bread prepared for Shabbat, together with a cooked food of high regard such as meat or fish, and giving these to another person by whom a share in this eruv is granted to the entire community. See Laws, page 605.
The one making the eruv says:

ERUV CHATZEROT
An Eruv Chatzerot (lit. a mixture of courtyards) was mandated by the Sages, to join numerous houses opening onto a single courtyard, thus making it possible to carry from one dwelling to another on Shabbat. Custom is to collect matzah from each house (this is customarily done on Erev Pesach) forming it into an eruv for every Shabbat of the year. See Laws, page 606.
He to whom the matzah is handed lifts it a tefach (approximately 3 inches), then gives it back to the one making the eruv, who recites the following:
Hebrew
בָּרוּךְ אַתָּה יְהֹוָה, אֱלֹהֵינוּ מֶלֶךְ הָעוֹלָם, אֲשֶׁר
