אֱלֹהֵינוּ וֵאלֹהֵי אֲבוֹתֵינוּ, קַיֵּם אֶת הַיֶּלֶד הַזֶּה לְאָבִיו וּלְאִמּוֹ,
וְיִקָּרֵא שְׁמוֹ בְּיִשְׂרָאֵל (פלוני בן פלוני). יִשְׂמַח הָאָב בְּיוֹצֵא חֲלָצָיו
וְתָגֵל אִמּוֹ בִּפְרִי בִטְנָהּ, כַּכָּתוּב: יִשְׂמַח־אָבִיךָ וְאִמֶּךָ, וְתָגֵל
יוֹלַדְתֶּךָ: וְנֶאֱמַר: וָאֶעֱבֹר עָלַיִךְ וָאֶרְאֵךְ מִתְבּוֹסֶסֶת בְּדָמָיִךְ,
וָאֹמַר לָךְ בְּדָמַיִךְ חֲיִי, וָאֹמַר לָךְ בְּדָמַיִךְ חֲיִי:

Eloheinu veilohei avoteinu, kayem et hayeled hazeh le'aviv ule'imo,

Our God and God of our fathers, preserve this child to his father and mother, and let his name be called in Israel (baby’s name son of father’s name). May the father rejoice in the issue of his body, and the mother be glad with the fruit of her womb, as is written, “May your father and mother rejoice, and she who bore you be glad.” And it is said, “Then I passed by you and saw you downtrodden in your blood, and I said to you: In your blood, live; and I said to you: In your blood, live.”
