CONFESSION BEFORE DEATH

The following confession is said by one near death. He or she should be told: "Do not fear. Many confessed their sins, and then recovered. Everyone who does confess has reward in the World to Come" (Shabbat 32a).

מוֹדֶה אֲנִי לְפָנֶיךָ, יְהוָה אֱלֹהַי וֵאלֹהֵי אֲבוֹתַי, שֶׁרְפוּאָתִי וּמִיתָתִי בְּיָדֶךָ. יְהִי
רָצוֹן מִלְּפָנֶיךָ, שֶׁתְּרַפְּאֵנִי רְפוּאָה שְׁלֵמָה, וְאִם אָמוּת, תְּהִי מִיתָתִי כַּפָּרָה
עַל כָּל חֲטָאִים וַעֲוֹנוֹת וּפְשָׁעִים שֶׁחָטָאתִי וְשֶׁעָוִיתִי וְשֶׁפָּשַׁעְתִּי לְפָנֶיךָ.
וְזַכֵּנִי לְעוֹלָם הַבָּא הַצָּפוּן לַצַּדִּיקִים. תּוֹדִיעֵנִי אֹרַח חַיִּים, שֹׂבַע שְׂמָחוֹת
אֶת־פָּנֶיךָ, נְעִימוֹת בִּימִינְךָ נֶצַח׃
Sephardi
Modeh ani lefanecha, Adonai Elohai veilohei avotai, sherefuati umitati beyadecha. Yehi
ratzon milefanecha, sheterapeini refuah shelemah, ve'im amut, tehi mitati kapparah
al kol chata'im va'avonot ufsha'im shechatati veshe'aviti veshepasha'ti lefanecha.
vezakeini le'olam haba hatzafun latzadikim. todi'eini orach chayim, sova semachot
et-panecha, ne'imot bimincha netzach.
English
I acknowledge before You, LORD my God and God of my ancestors, that my cure and my death are in Your hands. May it be Your will to send me a perfect healing. Yet if my death is fully determined by You, I accept it in love at Your hand. May my death be an atonement for all the sins, iniquities and transgressions I have committed before You. Grant me of the great happiness that is stored up for the righteous. Make known to me the path of life, in Your presence is fullness of joy; at Your right hand bliss for evermore.
Ps. 31

אֲבִי יְתוֹמִים וְדַיַּן אַלְמָנוֹת, הָגֵן בְּעַד קְרוֹבַי הַיְּקָרִים, אֲשֶׁר נַפְשִׁי קְשׁוּרָה
בְּנַפְשָׁם. בְּיָדְךָ אַפְקִיד רוּחִי, פָּדִיתָה אוֹתִי יְהוָה אֵל אֱמֶת׃ אָמֵן וְאָמֵן.
Sephardi
Avi yetomim vedayan almanot, hagen be'ad krovai hayekarim, asher nafshi keshurah
benafsham. beyadcha afkid ruchi, paditah oti Adonai Eil emet: Amen veAmen.
English
Father of orphans and Justice of widows, protect my cherished family, whose souls are bound with mine. Into Your hand I entrust my spirit. May You redeem me, LORD, God of truth. Amen and Amen.
When the end is approaching, the following should be said.
Repeat three times:

יְהוָה מֶלֶךְ, יְהוָה מָלָךְ, יְהוָה יִמְלֹךְ לְעוֹלָם וָעֶד.
Sephardi
Adonai melech, Adonai malach, Adonai yimloch le'olam va'ed.
English
The LORD is King, the LORD was King, and the LORD will be King for ever and ever.
Repeat three times:

בָּרוּךְ שֵׁם כְּבוֹד מַלְכוּתוֹ לְעוֹלָם וָעֶד.
Sephardi
Baruch sheim kevod malchuto le'olam va'ed.
English
Blessed be the name of His glorious kingdom for ever and all time.
