(פְּלוֹנִית בַּת פְּלוֹנִי) אֲשֶׁר נֶאֶסְפָה אֶל עַמֶּיהָ. זְכוֹר לָהּ צִדְקָתָהּ אֲשֶׁר
עָשְׂתָה, וִיהִי שְׂכָרָהּ אִתָּהּ, וּפְעֻלָּתָהּ לְפָנֶיהָ. אָנָּא הַסְתֵּר אֶת נִשְׁמָתָהּ
בְּצֵל כְּנָפֶיךָ, הוֹדִיעֶהָ אֹרַח חַיִּים, שֹׂבַע שְׂמָחוֹת אֶת פָּנֶיךָ, נְעִימוֹת
בִּימִינְךָ נֶצַח, וְתַשְׂבִּיעַ לָהּ מֵרֹב טוּב הַצָּפוּן לַצַּדִּיקִים.
Sephardi
(Pelonit bat Peloni) asher ne'esfah el amehah. Zechor lah tzidkatah asher
asetah, vihi secharah itah, ufe'ulatah lefanehah. Ana haster et nishmatah
betzel kenfecha, hodiehah orach chayim, sova semachot et panecha, ne'imot
bimincha netzach, vetasbia lah merov tuv hatzafun latzadikim.
English
(name daughter of father's name) who has been gathered to her people. Remember the righteousness that she did, and let her reward be with her, her recompense before her. O shelter her soul in the shadow of Your wings. Make known to her the path of life. In Your presence is fullness of joy, at Your right hand bliss for evermore. Bestow upon her the great goodness that is stored up for the righteous.

Utehi petirat hayeled hazeh ketz lechol tzarah vetzukah le'aviv ule'imo.
English
May the death of this boy mark the end of all anguish and sorrow for his parents.
For a young girl, add:

וּתְהִי פְּטִירַת הַיַּלְדָּה הַזֹּאת קֵץ לְכָל צָרָה וְצוּקָה לְאָבִיהָ וּלְאִמָּהּ.

English
Strengthen and support them in the day of their sadness and grief; and remember them (and their children) for a long and good life. Put into their hearts love and reverence for You, so that they may serve You with a perfect heart; and let their end be peace. Amen.
